Our friendly customer service representatives can answer most questions you may have about your policy if you don't find it in our Frequently Asked Questions (FAQ) section. Most service requests do require a form signed by the policy owner. Visit our Form Center to download the appropriate form for your request. For your protection, we ask that the owner's signature be witnessed by a Notary Public. We process requests in the order they are received and generally within five (5) business days.

We also accept requests via e-mail and fax. Please allow time for mail and processing, but we are always happy to check on the status of your request. Just call 1-800-438-7180.
