How to File a Medicare Supplement Claim


We have automatic claim filing, so all you have to do is show your Medicare Supplement ID card to your health care provider. Your health care provider will electronically submit your claim to Medicare and to our Medicare Supplement claims service center.

Lincoln Heritage Life Insurance Company has entered into an agreement with Wakely and Associates, Inc. and Insurance Administrative Solutions, LLC, third party administrators, to process all of our Medicare Supplement claims. Medicare Supplement claims should be filed in the following manner:

Part A claims should be submitted to:

Part B claims should be submitted to Medicare by your health care provider. These claims will be automatically filed with us.
